This book is based on a PhD thesis completed at Durham University in 2012. I cannot express enough my gratitude for Bronwyn, my wife and true companion of fifteen years, who supported and loved me all the way through the trials and joys of the three years that began when we uprooted our family and moved half- way around the world from Sydney to Durham.
